Dr. Ted Benke (pronounced Ben-key) specializes in the sensitive area of the body that starts at the base of the neck and moves up to encompass all the delicate workings of your ears, your nose and your throat. That's how the term "ENT" came about. It stands for Ear, Nose & Throat. Those are the areas treated by an Otolaryngologist.

What most people don't know is that an ENT physician also diagnoses and treats patients for snoring, allergies, and sleep apnea in addition to all forms of sinus problems, earaches, and hearing loss.

In addition, Dr. Benke is a member of the American Academy of Facial Plastic and Reconstructive Surgery and a Fellow of the American College of Surgeons. Treating the area of the head and neck also involves treating the skin that covers this vital area, and the cartilage that supports it. Dr. Benke can, for example, change the shape of your nose if you don't happen to like it, or he can repair damage to your nose if skin cancer has harmed it.

Dr. Benke's practice provides the added comfort of knowing that the physician who diagnoses your problem is also a surgeon who can correct it, saving you trips out of town and multiple sets of records. This is especially helpful when children and older adults are involved.

Appointments up^

We realize your time is valuable and will do our best to keep the amount of time you spend waiting to a minimum. Please arrive at least 5-10 minutes early to allow us to verify insurance information if needed and to enter information into the computer.

If you have already set an appointment, you may wish to click here to download and print out the Patient Registration Form, Medical History Form, Review of Systems Form, and Privacy Form, so that you may complete these before arriving at our office. Please bring these completed forms plus your insurance card with you at the time of your visit. Or you may FAX this information to us at (817) 641-3754 or mail it to the above address.

If for any reason you are unable to keep your appointment or wish to change the time, please call our office at (817) 641-3750 as soon as possible.

Insurance up^

We do participate in most managed care plans. Before your visit please (1) verify that we participate or are "in network" for your plan and (2) have your Primary Physician get a referral number if your plan requires one. Unfortunately, your managed care plan will not allow us to obtain the referral number for you.

Bring your insurance card with you to your office appointment. Co-pays or deductibles are due at the time of your visit.

Medicare up^

We will file your Medicare claims for you. However, we require that at the time of your visit you pay your deductible and co-insurance of 20% of Medicare's allowable fee. If you have met your deductible or have secondary insurance that pays your deductible you usually will not owe anything at the time of your visit.

Payment up^

If you do not have insurance to cover your office visit, payment will be due at the time of your visit. We do accept MasterCard, Visa, Discover and American Express.
